MEDICATIONS
Lose weight treatments

For years medical professionals told their patients to eat less and exercise more. As our medical knowledge  has progressed we know that this advice is simply not good enough. The main cause of obesity is genetic: if we are genetically pre-programmed to be overweight to lose and maintain weight, you have to act against your own body’s pre-programmed mechanisms. No wonder most of us find it hard to maintain weight loss.

Other common reasons for gaining weight include certain diseases, stress, sleep deprivation, and menopause. Certain medications can cause weight loss such as medications for diabetes, steroids and mental health medications. There are other causes of obesity which are being currently the subject of research. Plastic pollutants, certain viruses and bacteria might also be linked to obesity.

Why you shouldn’t wait to start treatment weight loss treatment?

I am a General Practitioner and I have undertaken further study to become an expert in weight loss treatment (please see: about me if you’d like to find out more). I would like to help you with your weight loss journey now. I believe it is such an important problem that you should not wait until you potentially develop complications of obesity (see: complications of obesity).

How can we treat obesity?

I see it day in and out – due to extra weight my patients develop trouble breathing,  widespread pains and aches, leg swelling, skin problems. As doctor I particularly worry about risk of strokes, heart attacks and cancers. I know obesity can affect a person’s self-esteem and even cause depression.

I will work with you to try to help. When we first meet I will ask you questions about your medical history and check your general health to make sure no other diseases contribute to the problem. If we find any other diseases which need treatment such as thyroid trouble, high blood pressure, high lipids or sugars we will treat them.

Depending on your wishes we will discuss range of treatments.

I can provide you with lifestyle advice how to lose weight and improve your health without any medication. We can talk about diet, underlying health problems that we need to treat first and ways to reduce body weight. Physical activity is important too, but mainly so you maintain the strength of your muscles while losing weight.

  • You must be over 18 and if female area not planning to become pregnant or breastfeeding;

  • Your BMI is 30 or over or BMI at least 27 with weight related health conditions (please see section on complications of obesity for details).

  • There are no medical contraindications for prescribing medical treatment for weight loss

MOUJARO (TIRZEPATIDE)

How this medication works?

Mounjaro is a medicine that contains an active substance called tirzepatide . It is used for treatment of type 2 diabetes and weight loss. Mounjaro works by regulating your appetite, giving you a sense of satiety (‘fullness’), making you feel less hungry and experience less food cravings. This helps you eat less food and reduce your body weight.

What are the site effects of medication?

When starting treatment with Mounjaro, in some cases you may experience dehydration, e.g. due to vomiting, nausea or diarrhoea. It is important to avoid dehydration by drinking plenty of fluids.

Other side effects include:

  • Low blood sugar
  • Nausea and vomiting
  • Diarrhoea and constipation
  • Abdominal pain
  • Inflamed pancreas (acute pancreatitis), which could cause severe pain in the stomach and back, which does not go away. It is a rare but severe side effect. If you experienced abdominal pain like this please seek medical advice without delay.

Who cannot use this medication?

  • Ladies planning on becoming pregnant or breast feeding
  • We need to be careful if you have severe problems with food digestion or food remaining in your stomach for longer than normal (including severe gastroparesis) or as a result of previous stomach surgery.
  • If you have ever had pancreatitis (inflammation of the pancreas which may cause severe pain in the stomach and back which does not go away).
  • If you had thyroid cancer
  • We have to be careful if you take certain medications for diabetes and diabetes is not well controlled as there is a risk of low sugars (hypoglycemia)

How to inject Mounjaro?

Mounjaro is injected under the skin (subcutaneous injection) of your stomach area (abdomen) or upper leg (thigh) Please use different injection site each week.

What to do if I missed the dose?

  • If it is 4 or less days take it as soon as you remember. Take the next dose at the usual day.
  • If it is more than 4 days skip the missed dose and administer the next dose on the regularly scheduled day.

Does Mounjaro affect oral contraceptive pill?

Some evidence suggests a reduction in protection by the oral contraceptive pill after starting treatment with Mounjaro, including after a dose increase. Therefore, you take oral contraception it is best to use alternative measures of contraception as an additional precaution. It is best to stop this medication for at least 2 months before trying to get pregnant.
MOUNJARO

WEGOVY (SEMAGLUTIDE)

How this medication works?

Wegovy is a medicine for weight loss and weight maintenance that contains the active substance semaglutide. It is similar to a natural hormone called glucagon-like peptide-1 (GLP-1) that is released from the bowel after a meal. Wegovy works by acting on receptors in the brain that control your appetite, causing you to feel fuller and less hungry and experience less craving for food.

When starting treatment with wegovy, in some cases you may experience dehydration, e.g. due to vomiting, nausea or diarrhoea. It is important to avoid dehydration by drinking plenty of fluids.

What are the site effects of medication?

  • Low blood sugar
  • Headache
  • Nausea and vomiting
  • Diarrhoea and constipation
  • Kidney or bladder stones. Signs may include back or lower abdomen pain, difficulty in urination or change in colour of your urine.
  • Inflamed pancreas (acute pancreatitis) which could cause severe pain in the stomach and back,which does not go away. It is a rare but severe side effect. If you experienced abdominal pain like this please seek medical advice without delay.

Who cannot use this medication?

  • Ladies planning on becoming pregnant or breast feeding
  • If you have ever had pancreatitis (inflammation of the pancreas which may cause severe pain in the stomach and back which does not go away).
  • If you had thyroid cancer
  • If you have untreated gallstones
  • We have to be careful if you take certain medications for diabetes and diabetes is not well controlled as there is a risk of low sugars (hypoglycemia)

What to do if I missed the dose?

  • If it is 5 days or less since you should have used wegovy, use it as soon as you remember. Then inject your next dose as usual on your scheduled day.
  • If it is more than 5 days since you should have used wegovy, skip the missed dose. Then inject your next dose as usual on your next scheduled day. Do not take a double dose to make up for a forgotten dose.

ORLISTAT

How this medication works?

Orlistat is a tablet that is designed to reduce absorption of fat in your meals.

What are the site effects of medication?

  • Common site effects include excessive wind, looser and more frequent stools
  • Some patients might experience bleeding from back passage or
  • Diverticulitis (inflammation of the bowel), hepatitis (inflammation of liver) and pancreatitis (inflammation of the pancreas). Symptoms might include upper or lower abdominal pain, change in your bowel habit, nausea and vomiting and fever. If you worried about inflammation in your bowel of pancreas you should seek medical attention without delay.
  • Kidney or bladder stones. Signs may include back or lower abdomen pain, difficulty in urination or change in colour of your urine.

Who cannot use this medication?

  • Ladies planning on becoming pregnant or breast feeding
  • If you have cholestasis (liver disorder)
  • If you have problems absorbing food (chronic malabsorption syndrome)

Do not take Orlistat if you are taking these medications:

  • Ciclosporin
  • Blood thinners such as warfarin

How to take this medication?

Tablet should be taken immediately before, during a meal or up to one hour after a meal with your 3 main meals of the day. The capsule should be swallowed with water. If you miss a meal, or your meal contains no fat, do not take orlistat.

As this medication can affect absorption of vitamins it is a good idea to take multivitamins at the bed time when you’re taking orlistat.

How to take this medication?

  • If it is less than an hour since your last main meal, take the missed capsule. 
  • If it is more than an hour since your last main meal, do not take the missed capsule. Wait and take the next capsule around your next main meal as usual.

I am a General Practitioner and SCOPE (World Obesity Federation) certified doctor who specialises in obesity management.

I graduated from Cardiff University Medical School and have completed BSc (Hons) in Medical Sciences with a first class degree at the University of Glamorgan (now the University of South Wales).  I also have a post graduate Certificate in Medical Education from Bangor University.

I have trained at NHS hospitals across North and South Wales. I have experience in internal medicine before training as family doctor and primary care physician. I work at NHS GP surgeries and the General Practice Out of Hours Service.

I worked as lecturer and researcher at Cardiff University. I have keen interest in medical research in the fields of diabetes and epidemiology and antibiotic treatments. I have presented work at medical conferences in the UK and abroad. I have published papers in medical journals, including the British Medical Journal.
